The home and community care sector is evolving and growing in order to better meet the needs of an ever-changing population. With a commitment to continuous quality improvement, the OACCAC and CCACs work together developing provincial innovations in care and approaches to their implementation.

Policy & Research

The OACCAC supports the contribution of Ontario's CCACs to the health-care policy landscape by developing papers, submissions and reports on health policy, and by providing information and analysis on legislation and regulation that affect CCACs and the delivery of home and community care.

Enabling Care Through Technology

CCACs are the only part of the Ontario's health-care system with single province wide e-health platform. This provides all CCACs access to common information and the ability to leverage technology to support strong connections and effective communication between home care, hospitals and primary care.


In 2012/13, CCACs served more than 653,000 people across Ontario
