These ongoing efforts ensure patients receive timely, high-quality care and help us to support our members and partners to work together more effectively and efficiently.

Together with the CCACs, we have identified shared priorities to help keep us focused on what really matters to the people of Ontario.

ENABLING CARE THROUGH TECHNOLOGY

Through a variety of technology-based programs and supports, the OACCAC enables CCACs to focus on delivering quality, timely care to their patients. These innovative initiatives include our full suite of Shared Technology Services for members and partners, and publicly-accessible information and referral resources, such as thehealthline.ca.

HEALTH LINKS

CCAC Care Coordinators work hand in hand with care partners to ensure patients' needs are met throughout their full care experience, particularly when moving from one part of the health-care system to another. Health Links is an innovative collaborative approach that brings together health-care providers in a community to better coordinate the care needs of high-needs patients.

INFORMATION & REFERRAL

Working with health-care system partners, Ontario's CCACs get people the right care, at the right time, in the right place. Through our information and referral (I&R) mandate, CCAC staff provide timely and accurate information to patients, family members, and other partners about available community and health services – electronically, by telephone, and in person. The website www.thehealthline.ca also offers a one-stop shop to help people connect to the services they need in their area.

NURSING INITIATIVES

CCACs are hiring more nurses to provide  front-line care to our most vulnerable patients including: frail seniors and adults; children with complex illnesses; and patients who require end-of-life care. These new initiatives include mental health and addiction support for students, rapid response nursing care for high-needs patients transitioning home from hospital and palliative care support.

OUTCOME-BASED CARE

Outcome-based care is a team-based approach to patient care that uses most current evidence to support decision-making about the care of each patient.  This model focuses on achieving specific outcomes for people with similar care needs - such as wound healing or better mobility - following surgery to the hip or knee. Work is underway to design, test and implement standardized care pathways that provide guidelines for care. These pathways may also link the payment for services to the achievement of specific health outcomes for patients.

PHYSIOTHERAPY

CCACs are now the single point of access for all publicly funded in-home physiotherapy services. This change to physiotherapy delivery will help to streamline care and promote access to physiotherapy for all recipients of these services.
